RELEASE: We recognize the possibility of physical injury associated with the sport of basketball. The coach’s signature below certifies that he/she has consulted with the parent or guardian of each player listed above and that the parent or guardian releases the Fireball Tournament Directors and their coaches, agents, representatives and all gyms involved where games are played.

NOTICE: Changes to roster must be given to tournament director in writing 2 hours before first game. No other roster changes will be allowed. Every player must be eligible to the 2009 National AAU Rules and have a birth certificate to play in tournament.
